New York State New Prescription Forms

Please note that as of April 2006 you MUST use official New York State Prescription Pads for ALL prescriptions!

The NYSDOH has enlisted NYSSPA's help to inform PAs that they must comply with this new regulation or they will not be able to write prescriptions in NYS!

FACTS:

1.   All practitioners as of April 2006 will need to have New York State Official Prescription Pads to write any prescription.

2.    This is true regardless of whether you are writing for a controlled substance or not, and even if you don't have a DEA number.

3.   PAs by law are required to have their supervising physician also listed on the pad. You will be able to order them that way but as a temporary measure you may stamp the physician's name and license number on your script (if you order them with only your name on them).

4.   It will take a few steps to get your new scripts. Your supervising physician must also be registered in order to complete the process of having both names on the script.

The following steps are also outlined on web site : www.health.state.ny.us/professionals/narcotic/ordering.htm. This includes downloads of the necessary forms. Please note this is a lengthy process. You will have to send forms by mail twice, one of them notarized, and you will have to email and receive email more than once to complete the process. The final ordering of the scripts can be done online once you and your physician are registered and you AND/OR the physician has an HPN number.

HOW TO OBTAIN YOUR SCRIPTS:

A. The first step for all providers is to fill out the attached form entitled "Official New York State Prescription Registration Form." It is also available to download at www.health.state.ny.us/professionals/narcotic/ordering.htm. The form must be mailed to the DOH as they require an original signature. If you do not have a DEA number, please call 1-866-772-4683 to complete your registration. TIP: The address on your DEA will appear on your scripts unless you order the scripts through another practitioner and add your name to the script. Then both names and one address appear on the scripts. If you want your title (RPA) after your name on the script, put it after your last name on the registration form just like you would normally sign it. They don't add it automatically.

B. Next you will need to request an HPN Medical Professions account application. Physician Assistants will need to obtain an HPN number in order to get their prescription pads with their supervising physician(s) name(s), DEA numbers and office address on them. You will need a computer and email address to do this. TIP: Only one person in a group needs the HPN to get scripts with multiple practitioners on them.

C. The last step is to order the scripts on the web site https://commerce.health.state.ny.us/hpn/ using either your own or your supervising physician's HPN account and follow the directions. You can preview your scripts before entering the order to be sure they have on them the necessary information. TIP: If you want your DEA# printed on the scripts there is a box to check off that is easily missed (otherwise you will need to print it on each script that requires one). You can have multiple MD, PA and addresses on the script once everyone in a group is registered.
